We are a UK mailing company and need a simple webform to help us collect and clean data ready for sending information by post. Our requirement is an online form (ideally using Microsoft forms) to collect names and addresses for people who want to be sent information. The webpage address doesn’t need to held within our domain. Required fields: First name Last name Street 1 Street 2 City County/region Post code/ZIP Country We do not collect email addresses. We need to keep history of who we send it to. This should include date submitted. We want to limit this information to 1 send per address so cannot be requested twice by the same person/household. Need to exclude symbols and non characters (&@#*% etc), plus maximum field lengths and not allow empty fields. Text ...

I coded a 2-page webform that captures data and inserts into AWS MySQL database. That works fine! Also, I created some DB tables and stored procedures to assign the task to users based on fewest amount of tasks. That works fine! After inserting the data and having the task assigned to a user, I need to send an email message to the person that filled out the form (To) and (From) the person that was assigned the task. If I use AWS' and enter my host, smtp username and password, the email is sent when I specify the recipient, sender, subject and body, but I need it to be dynamic and pull from the database.
