Customize Wordpress Contact Form 7 plugin, Bootstrap

I have a new Wordpress website. I installed genesis framework as well. What you need to do is customize the contact form 7 plugin. You will customize the plugin to look like this: https://secure.web-loans.com/[login to view URL] So, basically it is a loan application form. The form to contain the same information as it is listed on the link. There won't be a database. All the leads to be submitted to my e-mail directly using amazon SES. The plugin will be the home page of the website. However, I will want you to also include my company name and some content at the top and my office information at the bottom. This is my other website www.ctmortgageloans.com. So, I want similar design with my logo at the top with a one line slogan. Requirements: 1. Experience customizing contact form 7 2. Bootstrap, the form needs to be responsive 3. Able to modify the content in the back end 4. Amazon SES experience to set up e-mail notification.
