Help - transferring money from a Canadian bank to EU bank

Hello. I'm looking for someone who can tell me what is the proper way of instructing a Canadian business to make a standard bank / wire transfer to a European Union bank. What information is necessary, so anyone can easily make a successful transfer? For example, is this form correct? Let's say it's somewhere on the invoice.
