CARE SURVEY

I am an US Army Reserve Chaplain looking to connect with the Soldiers within my unit to determine the perceived health of our organization and their personal health. I would like to administer, monitor, and graph a survey that measures the health or toxicity of our or any organization and the people who comprise that organization by having the people within that organization self-report their perceptions of the organization's health and their personal health. The results will be graphed on a chart similar to a target or dartboard and the organization will be assigned a health rating. Each individual will be assigned a color based on their health and a position on the chart (target) based on their perception of the organization's leadership and their peer's concern for their well-being. I will use the website [login to view URL] for people to log in securely and self-report and it has to be real time to capture sudden changes in the health of the organization or the individuals. As a US Army Chaplain, I have confidentiality and will be the only person who would know the names associated with the ratings and comments but everyone will have access to the graph depicting the organizations' health and the health of the individuals within that organization.
