Wordpress metabox customisation

Ive got an existing theme that id like to add a custom meta box to the backend of the allow me to create an html element in the view. For the full spec please see the attached pdf file. We will be using git (bitbucket) for version control, you will need to work on the project locally, and push your code back to the repo. I will need this job to be finished relatively quickly (3-4 days max) including iteration / feedback. If you've got any queries please let me know and id be happy to jump on skype to discuss.
