A form with database and admin login

Hello, I need a form with database and admin login. The features are: 1. The form will be two stages with picture attached. 2. The third stage will be a preview stage where the picture will be on top and the information details will be below and final submission. 3. The Fourth stage will be ackwnoledge page where application number will be generated, attached picture, and four form fields filled will be available for printout. 4. The last stage will be a payment page where payment slip number will be added by the applicant. 5. The admin area is where one can delete, printout all submitted application. It should have a login.
