Joomla, PHP, HTML and CSS

Finish the last 25% of a Joomla project that includes PHP forms, MySQL, XHTML and CSS. The client was promised a site in Joomla Modules and it was completed in a template that they cannot manage. It was also done poorly and need QA on XHTML and CSS. Files are attached for remaining items.
