LAMP: web forms, small mysql database

This project is fairly strtaightforward. We need a few web page forms and supporting scripts to allow clients to register with us for support, and then to help us track who is being supported and who is still waiting... The forms and scripts need to interect with a mysql database. This project will likely be expanded later on with more forms and reports. begin project/ Starts with web page for new customer signup. Customer enters customer name, address, phone, contact name, email, and desired username and password, clicks submit. Client record gets created in mysql. A tcp port (between 10,000-20,000) is randomly chosen and recorded as assigned to this client. Client connection script (this is a very simple script of a few lines. Template will be provided, and needs to be automatically modified only with the port that is chosen for this client) is generated and placed in download area. Download link is emailed to client. New client signup notification gets emailed to tech mailing list at ourwebsite.com. Sometime later, client runs script and connects to our server. Web app monitor (a script that needs to be created by you as part of this project) sees new tcp connection, consults database to lookup port number, emails us that such and such a client is waiting, logs connection time in client record in mysql, and updates Current Connections web page (also needs to be created by you as part of this project) that techs use to see who is waiting and what port they're on. This page is for internal use only and can be quick and dirty, so to speak, as long as it is accurate. Our tech connects to port, web app sees connection, emails tech list that such and such a client is picked up, and updates Current Connections page to show that this session is actively being worked on, and logs to client record in mysql. Tech calls client and discusses issues and fixes something. Tech uses web form (needs to be created by you as part of this project) to submit work report with summary info, technician name, and billable time to mysql. Report is logged into client record, then emailed to accounting. /end project Possible add on features for later: client can use web logon to view client record online. Can update address, email, etc., and see all history. Staff will eventually need various reporting options as well.
