Create a Mobile Website

DB Driven mobile web app. I have a mysql DB with 2 tables. I would like a mobile web app built using either sencha touch 2 or higher or PHP. The backend of the app to send/receive data from the DB I would like to be in PHP. In the app the customer will enter their First name, Last name, street, city , state , zip code, email, and phone number. Please see attached [login to view URL] They will then move to the quoting section. They should have the ability to enter up to 8 pictures each with a description field and a room dimension field. Please see attached scr1.jpg. After they have entered their information a email to the administrator should be generated indicating there is a quote request. The quote should resemble the attached Excel sheet in layout. To format the quote there should be an admin section that allows updates to all of the DB fields. I have added the table and field names next to areas to be mapped on the excel sheet. The quote will be sent to the customer via email. The email will contain a link that pulls up a quote that resembles the excel sheet in layout. When the customer upens the quote I would also like a button that they can press that allows them to print/save to PDF.
