design and build facebook design voting competition for our fan page (FBML)

We are planning on giving away an ipad in exchange for someone designing us a a new logo. I want a page within our fan page.. where users can upload their logos as jpgs into an album on the page.. then the 5 people that receives the most likes on their design goes into a final where we choose the best and then they win an ipad. so we need an upload module.. and also a terms and conditions for entry section also we dont want people to have to "allow an app" in order to use it . just simply log in, upload their design and get their friends to vote. But we want it so people have to like the page before they can enter the competition or vote
