Custom Web Form

I require a multi-page form utilizing PHP, Java, MySQL and Ajax. The form is required to have the following elements... - Form - Must be secure and transmit submission over https - All form functions must be handled via php, jquery & ajax - Multiple File Upload - The ability to limit file size - The ability to set file types - The ability for the user to add additional upload fields via ajax - Form Pages - The form will have multipages. - The form will have a Progress Bar based on page completion - The forms "Next" button will be disabled until all fields are vaildated - Form Fields - ALL Field types must be able to be styled via css - Fields must be able to have a tooltip - Fields must highlight when active - Fields must include inline validation - Fields may show/hide additional fields based on selections - Form Summary Requirements - The form must produce a confirmation page with summary details prior to being submitted. - The form must allow the user to edit the form contents from the summary page. - The form must utilize captcha in order to submit. - Form Submission - After submission the user will be re-directed to one of several completion pages. - The "Completion Pages" URL's must be under admin control as they may change. - The "Completion Page" redirect will be based upon selections the user made in the form. - Form Results - Must be sent to multiple email addresses set be admin - Must give user option of having a copy of the email forwarded to themselves.
