Get this Form to work - Required Fields not working

Hi- We have a form (link below) that needs to have some fields required. We tried one script that you will see in there but you can use anything you want. I just don't have time to figure it out. The fields may need to be renamed because it looks like they don't work in this script as they are named right now. I also want the fields to get sent in the order that they appear on the screen now. The formmail script sends everything in alphabetical order, so that is why I was naming those fields with numbers first. But those numbers seem to be making the required field script work improperly.
