Application retrieve sql record for editing

UPDATE - Added create sql statement attachment. Also not used for entering records only changing. Application that will retrieve a record from one sql table for editing. The sql table is a mailing list, each record has a unique mail id. This mail id would be entered into a ID field on the application. After enter id into mail id field a lookup into the sql table is done and the matching record is returned to the window for editing. If the record not saved and a new id is entered a warning box with "Do you want to save this record?". This warning box will have "Yes or No" buttons. Clicking yes will commit the record and move to the next record. When a record is saved 2 additional fields are updated "Date" (Datetime field) with the current date time saved, and Update with the letter Y. If no changes are made and the save button is pressed the date field is still changed to current date/time along with the Update field changed to Y.
