Easy PHP mailing, logging and tracking application

We want to develop an application to send email to our customers with a logging and tracking function. It is an email with a HTML body and some links to webpages of ours. We start from an excel file with the email addresses and the salutation. The application should automatically send the emails to the respective email addresses. Furthermore there should be a logging of the email that were opened. In the email we ask our customers to visit a webpage to fill out a questionaire. There is also a link to an information page. We would also want a logging of these visits and the duration of these visits. The questionaire is simple and the customer only has to select a few options and submit. The system should automatically keep track of the email adresses where the returned questionaires came from.
