Short Questionnaire App (for non-public use only) development

Need a user fun, fresh, user friendly UI/ UX for mobile to capture simple contact information at event. This app is for organization (non-public) use only (i.e. not for commercial purposes) Questionnaire questions to capture: 1. Gender: Male/Female 2. Name: (First, Last) 3. Age: 4. Best method of communication?: (whatsapp/Email/phone) 5. WhatsApp or Email entry field based on prior field Requirements: - Should have a beginning screen with message (admin can change message) and a start button - Should have an end screen with message (admin can change the message) and finish button (once a contact is finished entering their information the app resets to the beginning and is ready for the next contact entry) - Steps to complete (visual) should be incorporated - Each question should have a next button that once clicked show the next question. Transition between questions should be fun but not overwhelming. In other words, we want kids to fill out the form completely and have a smile on their face when they do. - Fun, colorful, GUI UI/UX - No Auto Spellcheck - basic Email validation to catch mistyping and ensure legitimate email address is captured - Easy type i.e. app keyboard should offer button to select icons for @gmail, @hotmail, @yahoo, @outlook, - database for the responses should be linked to a google Doc but also store data locally incase of lost internet connection - Optional: gamify the questionnaire in some simple way (we have ideas but other ideas are welcome) - Should have two pre-sets based on source: =====>Source 1: "School" - fun colorful GUI and UX with simple gamification as it will be used by 15-18 year old kids =====>Source 2: "Event" - Event pre-set should be same fun colorful GUI and UX but without the gamification - Admin should choose a pre-set and name the source (i.e. school name, event name) prior to starting questionnaire - Admin should be able to select pre-set as part of initiating a new questionnaire - Admin should be able to open a previous questionnaire and enter new contact via questionnaire - Multiple users allowable whether at same event with multiple devices or at different events using at the same time - for Android and iOS if no difference in expense.
