Android App. Simple Ap to Record a Driver in and out time and notification to another party

I need an Android Ap. (maybe iPhone that a truck driver can use to record in/out times at a location. The interface would be simple: I envision simple tracking metrics like unit number, time in, time out, location for each log, etc. Simple dropdown selection for certain events like stoppage, detention at a location, basic reporting metrics, possibly using the GPS location / check-in ability of the phone to detail locations? Some front-loading of addresses might be necessary. I have all that data. I also would like this data to be uploaded and sent to a central server so office personnel are notified of event changes & the ability to capture and analyze data over a period of time.
