Basic Web Site with Ability to Auto Email PDF's to me

I need a basic insurance website built and installed on my server that will: - Allow for the consumer to pick English or Spanish version - Have coverage pdf's and application pdf's (fillable)that the consumer can fill in with their information - After they fill the application out, have a "Submit Now" button that AUTOMATICALLY attaches the filled out PDF form to an email and sends the entire copy to my mail box. - Basic web site, page 1 would be to choose language, then they would be taken to a screen with either English or Spanish versions of 1) Summary of insurance coverage's 2) Other Forms 3) Rate Sheets 4) Fillable PDF of the actual insurance application. - Again, after the consumer fills out either language version of the application - they click "Submit Now" and it gets automatically emailed to me as a attached PDF in an email. The actual application that the consumer filled out. Also, in my email it would note the time and IP address that the application was submitted from. I will provide the English and Spanish version of all the forms and fillable PDF's. Also logo's etc. I want a user friendly site with design that is good looking and simple to use. Add some pictures or graphics for nice looking site. Money in escrow. I will need this program installed on my sever for me. Please give details in your bid on how you will accomplish this.
