Convert a Template to a Website

I am a psychologist. I have developed two questionnaires (50 -75 items each) using excel to enable clients to perform self assessments of their symptoms and skills using 0-10 scales. I want to have the questionnaires converted into web apps accessible at a small cost to patients who will be required to pay with credit card. The responses made by a client must be encrypted with the highest security and results automatically generate radar plots (depicting scores on how they see their symptoms/skills 'now' and 'desired level' in future) and a bar chart depicting the gap btw their actual versus desired scores. Results would be automatically emailed to myself.
