Write macro for excel

Need to extract lab data entered into a formatted excel sheet in one tab and put into a table in a different sheet in the same workbook. Data can not be linked as the formatted lab sheet needs to be cleared once data has been copied to table. In excel sheet uploaded the lab data is recorded in the template sheet. The data needs to be saved in the table in the historical data sheet. Have typed data from lab sheet into the table to demonstrate what I would like the output to be from the vba code.
