Transcribing Information From Videos and putting them in a spreadsheet

I have recorded videos of folders with customers information on it. I need someone who can look at the videos and take the information and place it in spread sheets. This is information about our customers that need to be put in the spread sheet. Here are the fields. We are a funeral home and so 1. the first information, that will be in the spreadsheet, will be the name of the deceased person that passed away. No parentheses on the deceases name. 2. The second field will be the date the person died. 3. The next of Kin, the person we will be calling on to set an appt. 4. How is the person related to the deceased: is the person their son, daughter, wife, cousin, friend, mother, father etc.. 5. The phone number of the person we are going to call 6. The address of the person who's house we are going to. 7. Below the deceases information without parentheses are the people who we put parentheses around their names. The first person is the deceased person who has no parentheses and the others below are people associated with the decease person, family members etc... their names go under the deceases name. Once all of the family members have been listed under the deceases name, then start a new decease person who all of their family members added below them. There are about 600 names that need to be transcribed. We will provide the videos that need to be transcribed.
