google play store app suspended show need to correct and website also correct need

Hi Friends, We have app “Brix Antivirus” and it is suspended by google play store and it show : Updates won't be sent for review until you address the following issues: • You need to declare whether or not your application contains ads. • You must add a privacy policy • You must add a privacy policy if your app requests to use sensitive permissions. • You can’t edit this app until you create a new app release declaring sensitive permissions. Learn more • Complete the Target audience and content section • You must complete the Data safety form 1 : We have source code of this app, need to set this app. We have apk also. 2 : in this apk database is used for activation and our database has been changed for activate – need to correct it. 3 : our website php is not open anything – need configure so it will work.
