A 24-year-old female comes to the office complaining of a headache for the past two months. The headache is pulsatile in nature and she often feels nauseous. For the past one week, she has also started vomiting about once per day. She is an accountant by occupation and claims to have a lot of stress at work. She is also thinking of going to an ophthalmologist because she has difficulty reading the newspaper and while working on her computer. She is sexually active with her boyfriend and has been taking contraceptive pills for past one year. There is no history of fever. Her BMI is 33 kg/m2. The rest of the physical exam is normal. Which of the following is most likely to be associated with her headache?
A. Rhinorrhea
B. Neck stiffness
C. Photophobia
D. Papilledema
E. Abnormal CT scan

Explanation
This patient is suffering from idiopathic intracranial hypertension, also known as pseudotumorcerebri. This disease is commonly seen in young and obese females. It is also associated with OCP use, vitamin A toxicity and venous sinus thrombosis. Patients present with chronic headaches due to raised ICP. They may also complain of nausea, vomiting, blurred vision and diplopia due to VI nerve palsy. Patients have a normal brain imaging and normal CSF except for raised opening pressure during a lumbar puncture. Fundoscopy reveals papilledema.
Incorrect option A: Cluster headaches are more common in men, are of short duration, and high in intensity. They present with nasal stuffiness, rhinorrhea, retroorbital pain, lacrimation and conjunctival injection.
Incorrect option B: Neck stiffness is a feature of meningitis. Meningitis can be bacterial, viral, fungal or due to mycobacteria. It presents with fever, headache, vomiting, photophobia, altered consciousness, confusion and abnormal CSF findings.
Incorrect option C: Photophobia is a feature of migraine headaches and meningitis. Migraine is classically a unilateral throbbing headache associated with nausea, vomiting, photophobia, phonophobia and luminous hallucinations. Some cases may have dysarthria, numbness and weakness.
Incorrect option E: Pseudotumorcerebri has normal CT scan findings. However, brain imaging is required to rule out other possible diagnosis like brain abscess and tumor.
