Clinical Governance Audit

As a pre-reg pharmacist I need to write up an audit on clinical governance in my pharmacy store. I need to base it around clinical governance and patient safety in the pharmacy I'm working in. This includes reviewing the near miss log and the monthly patient safety reviews conducted in store, what patient safety incidents have happened and been reported and what improvements have been made. This audit needs to be made up so the near misses the patient safety reviews and patient safety incidents all need to be made up to make a 4 page audit report. I've uploaded a guide that was supposed to be used to help prepare for the audit and leading the audit. I've also uploaded the quick guide for what to include into the report. Only someone who has worked in a pharmacy or works in a pharmacy would be able to write this report
