New word-animated intro for video

We are working with a chiropractor in the development of a new web site and welcome video. The web site is complete [url removed, login to view], and the video is done [url removed, login to view] The client has requested us to add a word-animated introduction to the video. Since we don't normally do video of any kind, we would be looking to your expertise for the creative of an introduction to this video.
Something in the range of a 10-15 second intro, no more than 20 seconds. The overall style and presentation of the intro should be professional, with an easy flow to how it reads. The following are the words to be included:

Second, we would like to have the names of the 2 individuals added to the slides. The name/title for the 1st individual appearing in the video:
