Web Form Style Layout

I have a project website that has 3 - 5 simple web forms, but they are built with the basic WP Contact Form 7 or similar, so the look and feel is basic. I need a Graphic / Web Designer to provide a redesign of these forms, in PSD first, to show me a new style look and feel. I would then need the final layout built in *clean* html, and responsive. I will lay my php code into the pages after you provide the basic html pages. I would need the final files in layered PSD files also. I would need to see past examples of styled web forms
