Java script and CSS for existing form

Hi, I have an existing contact form and login form, but I don't like the layout of it. I would like to change it to fit more the new layout of the website. It has name, email and password label and fields: I want to have a text in the field that disappears when clicked inside It also has 2 drop down selection menu and a submit button that I want to change the style of it. And the login is just a and email and password fields The result should look like the attached image, and the same thing for the login part It's a very easy project, I will provide the form's source code as well as the login source code.
