Set up Live Camera and Playback/download from NVR on web

An example of live cam website is https://www.ipcamlive.com.
We don't need all feature but only live feed facility of the above website. plz see [login to view URL] for RTSP/HTTP STREAM setup of camera.
Also we want to playback video stored in NVR.
Below is summary or our requirement
1.) Live stream from PTZ Camera(including) without any plugin
2.) Video playback/download from NVR without any plugin
3.) PTZ (PAN, TILT & Zoom) Functionality as given in Hikvision plugin.
4.) Support for next 3 month after successful implementation of our production server. (may be many more month).
5.) What will bandwidth usage on server for each LIVE CAM & Video playback/download?
6.) Also plz remember that you need to completely setup the entire application on our server.
7.) Also you will provide us complete software with source code along installation manual so that we can our self set up server by my software team.
I want to see at least 1 PTZ CAM & 1 NVR working completely on your server before giving confirmation.
Our preferred development platform is LAMP.
