Convert Word Document Form to HTML Form.
-- Overview--
We want to convert the attached Word Document form to a HTML form. When the form is submitted it will need to save the form content as a PDF file and then either save it to the server or email it.
The HTML form should be designed neatly as should the output PDF.
The form should have live client-side validation and tips as well as server-side validation.
--The Code--
• Must be clean and optimized for load speed;
• Written using CSS tableless design, written HTML / XHTML / CSS which adheres to current W3C standards;
• Resolve perfectly in all modern web browsers including; IE8 / IE7, Firefox, Chrome & Safari
• Code must be optimized

• Experience creating HTML/JS Forms
• Experience Creating scripts
• Advanced HTML & CSS & Javascript
--Communication Skills Required--
• Have good written English language skills
• Be highly contactable and responsive via email
• Use Skype for chat and calls
