Advice on WordPress modules to be used

I am looking for someone to provide advice on WordPress modules to be used. This will be on skype voice/text chat. I will need about 30 minutes of your valuable time to answer some of my queries.

Queries are

1. Which is a best survey/question-answer module to integrate on wordporess platform

2. How configurable is it? I need hundreds of questions entered in a question bank. Questions and answers held in a database

3. Depending on customer profile entered online I wish to flash different questions from question bank. How do I do it thru this WordPress module?

4. Accept response and store in DB for these questions from users. Most answers should be click radio button else selection from drop down.

5. We move to results page now. We will go back and match user response to already entered answers step2. Depending on answers matched I need to send an email with my findings to user.

6. There would be an area of site where user profile, questions, answers and emails sent should be stored
