Simple multipart form

I need a simple multi-part form (using php) created:

Part 1 : Customer enters monetary value (gift vouchger value) in a text box and then hits next

Part 2 : Form pre-populated with the value enter at part 1, then fields which will be given to the successful provider hits next and the contents of the complete form sent to given email address(es)

Simple - just basic layout as I will incorporate into the site design.

Budget is very low on this one, so would maybe suit a GAF'er who is new or has no feedback yet - honest, reliable feedback is always given.
