Build a simple web application

The web application is pretty straight forward. I would like to be able to use a simple GUI interface for returning a list using a given string search in another table.
The web application would be hosted on a LAMP server. Meaning PHP is available and I would suggest you program using PHP and use mysqli extension for connection to and manipulation of the database.
