This would be a VERY basic contact form set up in a pop up modal that is linked to a button on the site..

the button would say: Monthly debit order with a logo

and inside the modal pop up you would then have

Monthly debit order

This block needs to open an online form that asks for Name and Surname, Contact number and email address. It will say “Please contact me to sign up for a monthly debit order” and when they press submit an email will be sent to an address.

This is a pop up modal that needs to link to a button

the coding will be done in the template that has already been set up, coded and created.
