I need WordPress form like .docx file. File attached in project description. All forms field shouldn't be stable. I want it to be dynamic as well. Because in future we will add some additional fields as well.
Form visual appearance should be look like ---> [url removed, login to view]
Once user will submit form. Form data will store in WordPress database. and then staff user will be able to export form data as spreadsheet and then forward it to administrator for application approval. (for sending data to admin i already installed plugin and it's working fine.)
You just have to create form and then store it into WordPress back-end. My site has many different users with different role. Administrator got everything.
Staff users will just see form data and export it as spreadsheet and then send it to CEO(admin). There should be special field kind of "Refer ID" or Staff member user name. Or you can make drop down field and put all stuff members usernames. Once visitor will add specific number of code--> refer id or select username of staff member from drop down field and then that form just goes to that specific staff member back-end and admin back-end. nobody can see expect staff member and admin.
For example; Refer by ---> (x,y,z,) so i select z and then my all form data will be store in z (staff member) back-end. none of other staff member will be able to see expect admin and z.
There should be around 10 staff users and 2 admins. (and i also need access of creating and deleting staff members as well)
Normal user will just fill up form and they will be able to see the status of their form application approved, pending or rejected and they would be able to post comments reply comments. that's it.
When CEO(admin) will approve application and then normal user will get email or login into our website and will see the status of their application has approved.
