I have CRE Loaded 6.2 Patch 08 with a contrib Batch Print Center [url removed, login to view],2720/download,11298 installed. I want to create a new pdf output template for this contrib.

I want the template to fill out a form I posted as attacment filled_out_form.jpg. This is filled with our address, [url removed, login to view] and our account number,order_total and customer address.

all the yellow background is on the paper it self I only want to fill out the predefined fields. The form is in A4 format.
