Require a simple online form to be created.
Will have:
Customer details such as Name, Address, Phone Numbers, Email, DOB etc...
Will then ask the customer such questions. Such medical conditions? The answers will be in a tick box form.
We are trying to convert our Remedial Massage paper based form to an online form.
The form data will need to go into a MySQL database. Also require a button on the registration for to export all the data from the MySQL database into a locally stored CSV file.
The form will be used by inhouse personnel and not for the public. So simple and doesn't have to look fancy, purely a form to collect data.
Will provide a scanned copy of the current for to the person who wins the bidding.
Very simplre requirements, but we need it ASAP.
