Type about 500 addresses from PDF into Excel

We need to type the school's addresses from the PDF's page 10 to page 67. Each page has about 5 or 10 addresses... some more or less.

You can just type them into Excel. The column headings are as follows:
School Name
Principal
Address
City
State (always MI for all of them)
Zip Code
We do not need the telephone numbers or anything else.
I will require a sample after you type the first 5 or 10 to make sure we're on the right track.
