We want the following screens (same screen one shows dropdown option ) redesigned in a modern slick professional way. Essentially we want to keep the same buttons but with a different layout/look. we are open to any new changes/layouts so long as the main fields are retained. you have the option to add additional fields or options to include additional information if you wish.

My feeling on the layouts is that they're a bit too dark, and could benefit with more icons

trauma, elective and emergency are the only three types of cases we deal with

Winner will be picked by best entry , Winner will get job of redesigning the rest of screens and potential long term work.
