I need a logo and app icon design for the iOS mobile app StreakIt.

StreakIt is an iOS app that encourages users to achieve daily challenges using a streak-based incentive model. In other words, users would have a goal, say "Exercise 30 minutes", and every consecutive they complete that goal their streak would increment by one. If they fail to complete this goal their streak resets to 0.
This app is themed around improving lifestyle habits.

We are open to all suggestions that have a sleek and relatively minimalistic design (monotone logos only).

The logo has to be square, so it can be used as an app icon.

It would also be good if an alternative logo could be designed as a combination with the name. Still deciding between having the name as "StreakIt" and "StreakIT" so please provide both. So one free-standing square logo (without name) and one logo that is integrated with the name.
