Excel Document-level Visual Studio 2010 project to pull data from Ms SQL database

I looking for an Excel Document-level project able to pull data from a Microsoft SQL database and populate an Excel Spreadsheet with the results of a query. Expectations: 1-Piece of code (not compiled) developed in Vb.Net - Visual Studio 2010 - Excel 2010 Document-level project 2-The code should allow changing the Query Expression. For developing purposes you can use a sample query similar to the following: "Select order_id, customer_id, order_date Orders_Table where order_date between @DATE_START and @DATE_END" 3-This Excel 2010 Document-level project should have a user interface, most likely a new Ribbon tab so a user can enter a couple of parameters (dates) and a REFRESH button. Also there should have some sort of form to configure the connection string to the server and database name. 4-The results of the query should be sent to the first sheet of the document. If the sheet already has some data from the last time the query was ran, it should be cleared before the new results are sent to it. 5-Brief documentation explaining how to install, configure an use this tool.
