Simple form to generate txt file

I need a simple form that will have a few dropdown options and a few input options. Once you submit it it will need to look at an existing txt file(template) and input that data from the form into the fields needed on the txt file. Then it will need to prompt the user to save the file. Please note that the existing txt file must remain unchanged as it will be the template for all submitted forms. Attached is an example of a template document that it will read from and enter the data where it is needed. No need to make it look pretty. Just need it to function as needed.
