Online Questionnaire System

The aim of this project is to create an online questionnaire system that will allow a certain group of users (survey administrators) to create surveys that other users (correspondents) can complete online. Web-based surveys will be generated dynamically from the retrieval of questions stored on a database. Once a survey correspondent selects a survey to complete, the questions for the survey will be retrieved from the database and the survey will be dynamically generated. All the correspondent needs to do is complete a survey. The following points had to be achieved at the end of the project implementation: • Creation of user login functionality • Surveys must be created through a web interface • A unique survey URL should be generated for each survey created • A survey should be able to be marked as active/inactive and only active surveys should accept new answers • The system should support multiple answer questions (check boxes) • The system should support pre-defined drop-down selections. • The system should support multiple choice questions (radio buttons) • The system should support free text answers • The system should support questions marking as optional or mandatory • The system should not support jump conditions • The system should support welcome, survey finished and survey completed messages • The system must be secure against SQL injection attacks • The system must be secure against Cross-site Scripting (XSS) attacks • Only members must have access to the system (Non-members can only fill questionnaires) Functional Requirements: • Authentication of all users (excluding correspondents). • Questionnaire creation by survey administrator • Generation of statistical survey result report • Viewing of completed surveys Other functions aspects of the system • Provision of Internet client/server application that will allow users to connect via a login system in order to complete surveys and create surveys. • Survey Administrators should be able to modify survey questions and answers before it has been sent to users. • Inform users of any errors detected while using the system or writing to the database, loss of connection to the server, database, etc. • Survey Administrators should have the ability to delete created surveys. • System should protect user anonymity. • Querying Database for previous survey questions
