FreePBX IVR Survey Module

Hi, I have freepbx installed in a server and I want to make a survey after the end of the call to ask the customer for feedback regarding the quality of the atention provided (by transfer call or agent hangup call). My idea is to upload some questions and receive feedback from customers, something like “Please, rate the attention…” and save the result in a data base. Here, it would be cool that several digits were available, not just 0 to 9. It’s important to have a flexible interface where I can create the surveys with multiple levels and load the audio files with the questions. Possibility that the Client rates service by voice message, For example: We then thank them for their time and ask them to leave message about their experience. Here Client leaves a voice comment and/or presses # to hang up or just simply hangs up.

Besides that, I’ll need to export the results, maybe filter by date, to a csv file.

Features:

• Compatible with FreePBX and Asterisk ( it will work for us as well with some updating to the latest versions of FreePBX/Asterisk)

• Has to be a friendly add-on module so that we can install and use easily.

• Posibility of voice message in survey

• Allow multiple questions and range of response values.

• Allow transfer calls to Survey

• Better to have a feature to send out alert email to manager based on certain condition(for example, survey score is 1 or survey score is 5)

• Store results in MySQL (date and time, agent or extension that received call, survey result, phone number, time, etc.). Web interface to view reports and export results into csv. Easy way to listen to voice feedback.

• Changing the questions will not invalidate the data in the database

And finally, the source code must be provided.
