Develop Medical Practice Software/CRM

I require a coder to develop a self hosted CRM for a medical practice. Happy for one of the open source versions (SugarCRM, vtiger, Zurmo, Fat Free) to be used & adapted. Minimum requirements: - Clean simple graphical user interface with branding. - Patient data entry & ability to locate patient according to name/date of birth/hospital number. - Secure medical notes entry with the ability to create/use multiple templates & upload images/scans/reports. - Scheduling module for multiple types of interactions across different sites ie setting up clinic consultations or procedures. Compatibilty with Google Calender & Outlook. - Communication with patient & physician from within software: text/email reminders - Billing & invoicing module, raise & send invoices as well as receipts. - Allow integration of Insurance company billing codes & costs. Once found to be satisfactory, to be installed on my own server. Any initial troubleshooting & fixes to be carried out within the winning bid Full rights & ownership to be transferred to me on completion. 20% of the successful build will be held & transferred after the system has been used & tested for 3 months.
