Cookie-based dynamic content and auto reply/back-end email notification for lead capture form

Hi, This job is two-fold. 1. I have a few affiliates. I have given them all a unique tracking URL. The tracking URL re-directs to my company's homepage. This is what I need. When a visitor comes through an affilaite's tracking URL, the visitor should be shown a different homepage. That is, I need dynamic content, that changes according to the referral URL (the tracking link). You will need to accomplish this using cookies. When the visitor clears their cache, only then should they see the default homepage. But if the cache has not been cleared, then the visitor should ALWAYS see the dynamic content. 2. When the visitor that has been referred from a tracking URL fills out the lead capture form, then my team must be notified. I need all information collected from the form in an email notification. Name, email and phone number. I also need the Referral URL (the tracking link) so that I know which affiliate sent the visitor to my site. If the visitor came to my site directly (that is, never clicked any affiliate link) then Referral URL is not required in the email notification. At the same time, the visitor, after filling out the lead capture form, should receive an email instantly. An auto reply. I will give you the content of the email.
