I want a single page web form with GIS capabilities and dynamic action based on given answer to previous question in the form. I will share a flow diagram with people who can show a previous experience and example work on the subject. I want it to be written in PHP with Openlayers. KML file format should be readble by the form if user prefers to upload. Preferable I would like to use PostGIS server for form output record but Spatialite will be accepted.
