Simple Web Form to calculate distance

Develop one PHP form with 4 fields:
1. Postal Code (From) - {6 digits Text Field} --> Singapore Postal Code
2. Postal Code (To) - {6 digits Text Field} --> Singapore Postal Code
3. Is Return Trip? - {Checkbox}
4.. Distance Travelled (KM) - Text Field
User will enter a Singapore postal code in Fields (1) and (2). Select is Return Trip.
Distance between postal code (1) and postal code (2) will be displayed automatically in (4).
If Is Return Trip, then distance x 2.
