Simple website with mysql database, forms and report view

This project is to build a simple website that will contain only a few forms with many fields of different type to be checked - radio buttons, select field, text boxes, check bock etc - the forms will be much like quiz forms with many questions the phone operators will have to fill. This is one section of the website where the information will be collected. I need other section of the website where i will be able to view in list mode all of the entries (20,30 or 50 per page). I also need to order the information from the query result. I will need search and order function to be able to search fast the info i need. I will need AJAX function in particular search box - need to be able to see the results in the moment of writing the word or number. For example if i am looking a phone number and start typing 678 to see the results that mach this number directly before clicking, same when you write a word in google. I need the website protected with password because will be used as intranet with 5-10 people. I want to be able to see who created a particular entry, the date and hour. I want to be able to create new users when i need, to enable, disable and delete. I need to levels of users, one admin that can create, edit and delete and the normal users that only can create and edit. To be able to change the name of the fields in different language (the main website will be in Spanish, but will need English also). You will make it in English so i will need to translate it, you will provide this functionality - i will translate it after that. So there are only a few things - forms, view of entries with order function, search. With basic CSS design it is not for public, it´s for very small group of people. I will need to be able to update the project with new functionality, so please be sure the code is clear, or that you will be able to make changes in future.
