Information Gathering and Database Building for a Medical Startup

I'm Dan Campbell, engineer by trade, inventor, founder and President of ReneGait LC. A few years ago I broke my neck wrestling and paralyzed myself. I have since been developing engineering and business skills to help people in my situation (trying to regain function, health, and independence). ReneGait is the vessel through which I do that.
By the summer of 2018, I will be launching the first product. It is a therapy tool designed to democratize a form of physical therapy that is typically very challenging and expensive to perform. That's where this database building project comes in. This project is essential to my mission.
I need someone to help me identify neurological rehab clinics in the USA. Furthermore, there is a certain "type" of clinic that is most likely to welcome and use my first product. Such specificity renders traditional data mining techniques useless. It needs to be done the slow way: neuro clinics need to be identified with Google searches, and then called so that a few questions can be asked to determine f they are my "type."
There is no specific number of clinics I am aiming to find. I will pay per entry, and will personally screen submissions to ensure the information is current and accurate. At some point, after the effort to find clinics becomes too great perhaps, I will end the project.
All details can be found in the three attachments below. I will ask you to read and sign the Independent Contractor Agreement (attached) and a Confidentiality Agreement (not attached). The other two attachments are a snapshot of the actual Excel file database you will populate and the "key" document that walks you through how to get that information.
