This is not at all a word or excel job. Let me describe you what needs to do. - I will develop an application based on windows using asp.net - Application will accept the input as an image. So you need to upload scanned image. - I will use an open source third party library for OCR recognition. - You need to press next button which will redirect you to next form page. And I will fill the data there programmatically. I will wait for your message to discuss further.

Hello, I have done many projects in image/video processing area, especially face recognition and streaming on webcam (Kinect/IP /USB cams) as well as OCR text extraction. License Plate Recognition System Developing a realtime system that involves reading video from a capture device (IP Camera) then processes images captured to return plate numbers. Hockey Live Video Streaming on Wowza Tracking players and streaming encoded live video from a webcam to a Wowza server, as well as storing it into a media file on the local machine in parallel. 3.
