Create 15 Great Landing Pages for my Website PPC Campaign -- 2

I would like you to design 15 landing pages as well as 5 Thank You pages built within my website.

There is no ftp!! You must use my website hoster, webpage manager to build these pages. The pages you design cannot be uploaded using FTP.

I am running a PPC google and bing campaign. I want these landing pages to be simple, eye catching as well as have a clear call to action.

The actions I am looking for are:

Fill out their name and email address for something they want:

1) Schedule a FREE, no-obligation consultation (x3 types for A/B/C Testing)

2) Call for an appointment (x3 types for A/B/C Testing)

3) Download a FREE report on lower back pain (x3 types for A/B/C Testing)

4) Download a FREE report on neck pain (x3 types for A/B/C Testing)

5) Download a FREE report on headaches (x3 types for A/B/C Testing)

We are going to be testing these 3 pages of each type with associated Thank You pages encouraging the user to take another action (ie. Schedule)

We are going to see which one is more effective and getting the desired outcome.

You can use HTML, CSS & Java but only on the back end of the hosting company, webpage editor.
